TERMS FOR ALL COURSES AND MEMBERSHIPS
COURSE TUITION
The course tuition must be paid in full prior to attendance or the granting of customer access to the course. Any and all applicable taxes will be invoiced. The course tuition includes instruction; course materials; the use of software, computers and other technical hardware as appropriate to the course.
ONLINE COURSE/MEMBERSHIP CANCELLATION POLICY AND MONEY-BACK GUARANTEE
Core 4 Healing and Wellness, LLC includes a 30 day money back guarantee for online courses. If a customer would like to cancel their purchase after 30 days, they will forgo reimbursement for the course purchase. Core 4 Healing and Wellness, LLC hosts a membership site with automatic annual renewals charged to the credit card utilized by the customer to sign up for the membership program. This membership can be canceled at any time by sending an email to [email protected].
VIRTUAL PELVIC WELLNESS CONSULTATIONS AND IN-PERSON PHYSICAL THERAPY SESSIONS CANCELLATION POLICY
It is the responsibility of the customer to inform Core 4 Healing and Wellness, LLC by phone (570-847-8312) or email [email protected] at least 24 hours in advance if they will be unable to attend a scheduled virtual consultation or in-person physical therapy appointment. If the customer does not provide 24 hours notice, they will be responsible for paying the consultation fee in full and rescheduling their session if decided. Core 4 Healing and Wellness, LLC may make exceptions based on personal hardship or extraneous circumstances outside of the customer’s zone of control.
ACCOMMODATION FOR DISABILITIES
We are committed to complying with all applicable federal, state and local laws regarding reasonable accommodations for course participants with disabilities. Accommodations should be requested by a course participant, preferably in writing. In order to plan for and provide effective accommodations, we ask that accommodations be requested as early as possible, and no later than two weeks before the start of the course.
Core 4 Healing and Wellness, LLC will do its best to meet the requested accommodations of the course participant. Accommodations that fundamentally alter the nature of the services being provided, jeopardize safe operations, or result in undue hardship may not be available. Please email [email protected] to advise Core 4 Healing and Wellness, LLC of any need for accommodation.
SECURITY FOR ONSITE COURSES AND PHYSICAL THERAPY SESSIONS
The customer agrees that all course participants, patients, and clients will abide by security measures in effect at the clinic or course location.
COURSE POSTPONEMENTS FOR ONSITE COURSES
At times, we must change a course location, course date or postpone a course. We will try to give the customer as much advance notice as possible of any such change. If the customer is unable to attend a revised location or date, we will credit 100% of any prepaid course fees paid against a future course or, if requested, refund those fees.
COPYRIGHT/INTELLECTUAL PROPERTY
The customer can use any course materials or educational materials provided or made available in connection to our courses or our partners/vendors’ courses solely during and for the purposes of the course, and does not have any ownership interest or license to these materials for any other purpose, use, or time. All copyrights, patents, designs and other intellectual property rights in or relating to any course materials provided or made available in connection with our courses or our partners/vendors’ courses remain the sole property of the owner. No part of any course materials may be used for any other purpose, reproduced, stored in a retrieval system, disseminated, or transmitted, or published in any form or by any means, electronic, mechanical, photocopying, recording or otherwise, or translated into any language without the prior written permission from the owner.
INDEMNIFICATION
The customer agrees to defend, hold harmless, and indemnify Core 4 Healing and Wellness, LLC, its affiliates, officers, directors, employees and its partners/vendors against any allegations, investigations, claims, actions, demands, damages, expenses, fees (including attorney’s fees and court costs) caused by, arising out of, or related to customer’s or a third party’s unauthorized use, reproduction, dissemination, publication, transmission, or storage of any materials provided or made available to customer in connection with our courses or our partners/vendors’ courses, including but not limited to any alleged infringements of patents, trademarks, registered designs, design rights or copyright.
THIRD PARTY SERVICES/PRODUCTS
Core 4 Healing and Wellness, LLC may provide advertisements of third parties on our website and/or feature information, materials, products, or services provided by third parties. Core 4 Healing and Wellness, LLC makes no representations with respect to, nor does it guarantee or endorse the quality, non-infringement, accuracy, completeness, timeliness, or reliability of such third-party materials, information, products, and services you may access. Your correspondence and any other dealings with third parties on our website are solely between you and the specific third party or third parties. Core 4 Healing and Wellness, LLC has no control over and expressly disclaims responsibility and liability for all third party provided information, materials, programs, products, and services covered on or accessed through our website, and you agree that Pelvic Health shall not be responsible for any loss or damage of any sort incurred as a result of any of these dealings or as the result of the presence of such third parties on our website.

Our brochures and advertisements are for information purposes only and are not intended to form any agreement between Core 4 Healing and Wellness, LLC and the recipient. These terms and conditions are subject to change without notice and changes will apply to any enrollment/course order received after the date of the change. These terms and conditions may not be varied except in a written agreement signed by an officer of Core 4 Healing and Wellness, LLC. Core 4 Healing and Wellness, LLC acceptance of a purchase order containing different terms and conditions does not modify or supersede these terms and conditions.
ARBITRATION
Any controversy or claim arising out of or relating to this Agreement, or the breach thereof, shall be settled by final binding arbitration administered before a single arbitrator by the American Arbitration Association (“AAA”) under the rules of the AAA in force and effect at the time of the dispute, and judgment on the award rendered by the arbitrator may be entered in any court of competent jurisdiction. The AAA’s fees and charges shall be paid equally by the parties as they become due, provided that such costs shall be recoverable by the prevailing party. If either party fails to pay its share of the AAA’s fees or expenses as they become due, and such failure is not cured within five days of receiving written notice from the other party or the AAA, such party shall be deemed to have defaulted and the arbitrator shall enter final judgment in favor of the non-defaulting party.
GOVERNING LAW
This agreement will be governed by Pennsylvania law, without regard to any conflicts of law.
